Enrolment Forms

NOTE: Please make sure you have a suitable PDF program to open the PDF forms in the links below. If you don’t have a PDF program installed, you can download and install a free version of Adobe Acrobat Reader.

Please remember to uncheck the option to install McAfee Anti Virus software.

Also, please make sure your computer is set to open PDF documents with Adobe Reader, if your using Windows 10, search for “Default App” and open.

Select “Set default by app”

Hubworks (Online)

Each parent is required to complete and submit a Hubworks online.

Each parent is required to complete and submit a iPay Fee Schedule Form.

Please read carefully

IMPORTANT: The parent that has dealt with Centrelink in regards to applying with them for Subsidy MUST be added as Parent 1 on your Hubworks enrolment form.

Please ensure you have registered your child for Child Care Subsidy and have entered your Reference numbers (both parent & child) on your enrolment form.

If you haven’t done so as of yet please log in at some point before the New Year and do so.

Hubworks and Centrelink work together in providing us with your out of pocket payments. Failure to enter reference numbers before your child’s start date will mean you will have to pay full fees and wait to be reimbursed by Centrelink.(if eligible)

(please keep in mind this is an estimate from Centrelink and can change anytime by them).

Once your enrolment has been accepted you will receive an email from Hubworks with a username and password. You will then be able to LOGIN into your child’s account and make any changes as they arise throughout the year e.g. Immunisation, Contact Phone Numbers, Emergency contacts etc. You will also be able to view your own statements and monitor your payments and subsidy eligibility.

It is EXTREMELY IMPORTANT you keep us up to date with current information for the safety of your child, especially contact numbers.

Additional Forms (PDF)

PLEASE ATTACH A LETTER FROM THE CHILD’S DOCTOR TO THESE FORMS.

Please note: Prescription medicine will not be administered if labelled for anyone other than the child named on this form. Please ensure the medication (prescribed and over the counter) is clearly labeled with your child’s name, Date of Birth and dosage, frequency required. For more information refer to Medical Conditions Policy, Administration of Authorised Medication Policy located in the links below.

NB:ONLY medication that clearly states the child’s full name and date of birth will be administered.
